Always when i turn on the game and i play like 5 minutes or even less it crashes with err_gfx_d3d_init error. It says: Failed Initialization. Please reboot and restart

Window Mode I get the same freezing, but no ERR_GFX_D3D_INT, I instead get a white window with music playing for 1 minute then a error stating GTA 5 has stopped responding and needs to close. Regular Mode I get freezing, ERR_GFX_D3D_INT BSOD (Black Screen) soft crash and am able to recover to desktop and try again.

When I try and play My game crashes instantly with my AMD radeon hd 7700.
